Transition Care Program
The Transition Care Program is for Health and Ageing and Queensland Health, offering time limited, goal orientated flexible slow stream rehabilitation care packages to older persons who have had an acute or subacute hospital admission. It is primarily aimed at an older person (50 years or older for an Aboriginal or Torres Strait Islander person).
The Program aims to provide:
- short term support, therapy and case management (up to 12 weeks), to assist older persons transition from hospital to home.
- goal orientated, time limited support and therapy at the conclusion of a hospital episode of care.
- a client's functional capacity by supporting clients to further improve or maintain their physical, cognitive and psycho-social functioning.
Eligibility is determined by an assessment abiding by the aged care act and guidleines.
TCP accepts referrals from all public and private hospitals.
